Women who choose fertility treatment after one year of trying for a baby naturally may do more harm than good, a leading fertility expert has warned.

"Many young women would be better off improving fertility through simpler means such as cutting stress levels and losing weight, according to Dr Geeta Nargund.

Research shows those who opt for IVF after 12 months are only slightly more likely to conceive than those who let nature take its course. They are also more likely to have twins or triplets, raising the overall health risk.

Dr Nargund, head of reproductive medicine at St George's Hospital in Tooting, South London, told the Daily Mail: "We must offer lifestyle advice, early tests and simple approaches to promote natural conception. If a woman is younger and has no serious medical causes of infertility, she should consider improving her fertility naturally.

"IVF is safe but what we are saying is you don't want unnecessary medical treatment far too early.""

"More than half of women who die after having a child are overweight, as the obesity epidemic drives up the death rate among new mothers, according to a report."

"Obesity is one of the biggest problems facing fertility doctors and obstetricians because of the difficulty that overweight women have in conceiving and, later, the increased risks they face during pregnancy.

The Cemach report comes after the British Fertility Society called for more women to be told to lose weight before they have IVF in order to help them conceive naturally and give birth safely.

The NHS should not fund IVF treatment for women with a body mass index of more than 35, the society said.

Obese women are at greater risk of miscarriage and are more likely to need a caesarean section delivery, which has its own risks and complications.

They are also at increased risk of high blood pressure and blood clots, both of which can prove fatal. Babies born to obese mothers are more likely to suffer congenital abnormalities, be stillborn or die shortly after birth."

Pregnant women are at increased risk from obesity because of hormone changes during pregnancy - e.g. higher oestrogen levels. - Oestrogen is not just a sex hormone; it is a steroid, and in common with many other steroids, when its concentration in the blood increases, the walls of the veins relax and become more vulnerable to the incursion of extra salt/sodium and its accompanying water. - This extra water is, of course, a form of fluid retention: obesity is caused by fluid retention brought about by sodium/salt sensitivity. These pregnant women have become more sensitive to salt/sodium. This is why dieting does not work. The truth should be told - namely that to lose weight it is necessary to give up dieting and concentrate on reducing salt intake. To avoid unhealthy weight gain and many other health problems, it is very important that pregnant mothers keep their salt intake low.

"Twins have become the first babies born in Britain using a revolutionary new fertility treatment that does not require women to take potentially dangerous hormone drugs.

The boy and girl were born a week ago after they were conceived through vitro maturation (IVM) - a technique which is quicker, safer and cheaper than traditional IVF.

The twin babies, whose parents have requested that their identities not be released, were born a minute apart at the John Radcliffe Hospital in Oxford. Both are said to be healthy.

The boy, who was born first, weighed 6lb 11oz, while his sister weighed 5lb 14oz.

Although the new technique currently has lower success rates than IVF, doctors believe it could one day become the standard treatment for infertility.

The twins are the first babies born in the UK using the technique, however about 400 IVM babies have been born worldwide.

The Oxford Fertility Unit, the private clinic which announced the twins' birth, became the first clinic in the UK licensed to carry out the procedure earlier this year.

Mr Tim Child, a reproductive medicine researcher at Oxford University and a consultant gynaecologist at the Oxford Fertility Unit, said: "I'm absolutely delighted for the couple."

"Thousands of couples are being denied NHS fertility treatment despite a government pledge of a full IVF cycle for those eligible by April 2005, a Department of Health survey has found.

The survey, the first monitoring by officials of how local NHS provision measures up to national guidelines, reveals that more than half of England's health authorities fail to provide the most basic treatment of a full IVF cycle, including freezing and replacement of embryos not transferred in the initial cycle.

Many women and their partners who fulfil strict criteria for a free IVF cycle are being denied it altogether, while others are receiving partial treatment which falls short of guidelines set up to standardise provision.

A recent all-party parliamentary report found that at local level the NHS regarded treatment of infertile couples as a low priority, with trusts "reluctantly funding the barest minimum" of treatment, if at all. Primary care trusts have refused treatment to smokers, to those considered too fat or thin, and to couples who have had sterilisation procedures.

The guidelines, issued in February 2004 by the National Institute for Clinical Excellence (Nice), advise that eligible couples should be offered up to three cycles of IVF if the woman is between 23 and 39 years old. The measures were welcomed by John Reid, the then health secretary, who promised "at least one full cycle" to those eligible by the next year, with the NHS to work towards full implementation of three free cycles.

But the survey, by Infertility Network UK on behalf of the Department of Health, revealed an "enormous variation" in the social criteria set by trusts, with the female age range differing from the guidelines. It led to a situation where a 36-year-old woman in Wiltshire was told she was too old to have the treatment, while a 28-year-old in the Thames Valley heard she had to wait eight years for treatment because only those aged 36 to 39 were eligible."

High Demand for IVF

"The fortysomethings are the fastest-growing group of patients seeking IVF, up from less than 1,000 in 1991 to more than 6,000 in 2006.

Demand for IVF has soared for all age groups in the past decade, with almost 40,000 cycles of treatment provided in 2006. Forty-plus women have led the surge, rising from 10 per cent of all cycles in 2000 to more than 15 per cent in 2006.

Experts said the trend was a consequence of the social pressures on women to delay starting a family while establishing their careers and growing awareness of the potential of fertility treatment. But they warned that many older women faced having their hopes dashed as success rates for IVF declined sharply with advancing years.

The figures also reveal rising demand from single and lesbian women. Cycles of treatment provided to single women increased from 600 in 1999 to more than 1,200 in 2006. The number of treatment cycles for lesbian couples rose from 300 to almost 1,000 in the same period.

Medical advances and improvements in technique have seen success rates for treatment increase dramatically. The overall live birth rate has risenfrom 14 per cent per cycle of treatment in 1991, to 21 per cent in 2006. The success rates are even higher for women under the age of 35 with more than one in four becoming pregnant at the first attempt. But the rates fall sharply after women pass the age of 35, declining to 12 per cent at age 40.

Moreover, the over-40s have not seen the improvement in live birth rates experienced by younger women. Success rates have improved only slightly over the past 15 years as doctors have run up against the barrier of female biological clock. After the age of 43, more than 95 per cent of patients treated return home childless."

Obesity reduces fertility. "Women having IVF treatment may be restricted to having only one embryo implanted at a time to avoid the dangers of multiple births.

In a public consultation paper launched on Wednesday, the Human Fertilisation and Embryology Authority will raise the issue of “single embryo transfer” amid fears that multiple births are putting the lives of mothers and babies at risk.

Strict rules governing IVF treatment currently mean that women under 40 in Britain can have two embryos implanted while those over 40 are allowed three.

The more embryos that are transferred, the greater the chance of twins or triplets, increasing the risk of premature births, cerebral palsy, low birth weight and other complications. "


Most women can improve their fertility by reducing their salt intake. - This is because reducing salt intake reduces any fluid retention, which is the cause of obesity, and there is a positive correlation between obesity and infertility.
